Bertahun-tahun sudah Behmen (Nicolas Cage) dan Felson (Ron Perlman) hidup dalam peperangan. Insting mereka sudah terasah tajam sementara naluri mereka sebagai petarung juga tak bisa dianggap remeh. Sayangnya, kedua prajurit Perang Salib ini tak sadar kalau tugas yang bakal mereka hadapi ini ternyata jauh lebih mengerikan dari pertempuran yang pernah mereka alami.
Betapa kecewa Behmen dan Felson saat pulang ke kampung halaman mereka dan mendapati tanah kelahiran mereka hancur porak-poranda, bukan karena musuh namun karena wabah yang telah melanda tanah itu. Jarang yang bisa selamat dari wabah mematikan dan satu-satunya harapan yang tersisa hanyalah Behmen dan Felson. Gereja memerintahkan Behmen dan Felson untuk menangkap dan membawa seorang gadis muda bernama Anna (Claire Foy) yang dituduh sebagai penyebab dari wabah mengerikan itu ke sebuah biara agar wabah ini segera berakhir.
